Entrepreneurs kept by the District Court in Warsaw in Warsaw, XII Commercial Division of the National
Court Register under the number KRS 0000010681, NIP: 526-02-50-995, with fully paid-up share
capital of 3,937,072,437 PLN. covered in full, hereinafter referred to as the "OPL", which is the
organizer of the Contest.
OŚWIADCZENIE/ STATEMENT

Oświadczam, że znam i akceptuję warunki regulaminu konkursu BIHAPI, w brzmieniu
określonym na stronie www.bihapi.pl

I proclaim that I have read and accept the Terms and Conditions of BIHAPI contest, as set out on
page www.bihapi.pl

Administratorem danych osobowych jest OPL. Dane osobowe będą przetwarzane w celu
przeprowadzenia konkursu BIHAPI zgodnie z przepisami Ustawy z dnia 29 sierpnia 1997 r. o
ochronie danych osobowych, Dz. U. z 2002, nr 101, poz. 926 ze zm.

Personal data administrator is the OPL. Personal data will be processed in order to carry out the
BIHAPI in accordance with the provisions of the Act of 29 August 1997, Protection of Personal
Data, Journal of Laws of 2002, No. 101, item. 926, with later amendments

Wyrażam zgodę na wykorzystanie mojego wizerunku przez OPL w celach reklamowych,
promocyjnych i marketingowych związanych z konkursem BIHAPI.

I agree to the use of my image of the OPL for the purposes of advertising, promotion and
marketing of the contest BIHAPI.
5.
Zgoda wskazana w ust. 3 i 4 powyżej obejmuje takie formy publikacji jak:
1) publikację na stronie internetowej oraz w prasie drukowanej lub materiałach promocyjnych,
2) zamieszczenie w materiałach promocyjnych i informacyjnych OPL, a w szczególności
konkursu BIHAPI
i dotyczy fotografii przedstawiających moją osobę, udostępnionych przeze mnie OPL w formie
elektronicznej lub wykonanych w trakcie realizacji konkursu „BIHAPI”. Jednocześnie dopuszczam
możliwość przetwarzania mojego wizerunku poprzez kadrowanie i kompozycję.

Consent indicated in paragraph 3 and 4 of the above includes such forms as:
1) publication on the website and in the printed press or promotional materials,
2) inclusion in promotional and informational materials OPL, and in particular in the BIHAPI
materials
and relates to the photographs of my person, made available to OPL in electronic form or made
during the contest "BIHAPI". At the same time I admit the possibility of processing my image by
cropping and composition.

Wyrażam zgodę na opublikowanie opisów koncepcji Projektu i Prototypu na stronie
www.bihapi.pl.

I agree to the publication of descriptions of the Project and Prototype concept on the
www.bihapi.pl.
7.
Wyrażam zgodę na uruchomienie Prototypu w celach demonstracyjnych w środowiskach
sieciowych OPL.

I agree to run Prototype for demonstration purposes in OPL network environments.
8.
Wyrażam zgodę na udostępnienie Organizatorowi kodu źródłowego Prototypu (opcjonalne)**.
